Job Description

We are looking for a passionate Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and more.

Responsibilities

• Conduct psychosocial assessments for patients and families.
• Provide counseling and support regarding medical procedures, aging, discharge planning, and end-of-life care.
• Collaborate with healthcare teams to address patients' needs and concerns.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
