SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.

Responsibilities

• Offer clinical social work services and psychosocial assessments.
• Assist patients with adjustment to institutional living and changing circumstances.
• Provide counseling on various topics including financial concerns and family relationships.
• Engage in discharge planning and offer support related to death and dying.

Requirements

• Bachelor's degree or equivalent in social work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Familiarity with theory and practice within the social work field.
