SOCIAL WORKER PRN

Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker PRN, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing a range of common issues including adjustment to institutional living, financial concerns, family relationships, aging-related changes, and more.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Offer counseling on various issues such as discharge planning and death and dying.
• Assist patients and families in coping with medical diagnoses and treatments.
• Collaborate with a multidisciplinary team to ensure comprehensive care.

Requirements

• Bachelor's degree or equivalent professional knowledge.
• 1-3 years of experience in a similar role.
• LBSW/LMSW/LGPC certification.
• Strong understanding of social work theory and practice.
