SOCIAL WORKER PRN

Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD, based at Levindale Hospital. As a Social Worker PRN, you will provide essential clinical social work services and psychosocial assessments to patients, residents, and their families, addressing a wide range of common issues. From adjustment to institutional living to discharge planning and counseling related to death and dying, you will play a critical role in supporting individuals in challenging situations. This position is perfect for someone who is compassionate, experienced, and committed to making a positive impact on the lives of others.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients, residents, and families.
• Address common issues such as adjustment to institutional living, financial concerns, and family relationships.
• Offer counseling related to medical diagnoses, changes relating to aging, and end-of-life care.
• Participate in discharge planning and empower individuals in difficult situations.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience as a Social Worker.
• Licensed as LBSW, LMSW, or LGPC.
• Strong understanding of social work theory and practice.
