SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families. Your responsibilities will include addressing a variety of common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common problems faced by patients and families.
• Assist with discharge planning and counseling related to aging, medical conditions, and end-of-life care.
• Collaborate with healthcare teams to ensure holistic patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in a healthcare setting.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
