SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients and families, addressing various common challenges such as adjustment to institutional living, financial concerns, and discharge planning.

Responsibilities

• Conduct psychosocial assessments for patients and families.
• Address issues related to aging, family relationships, and medical procedures.
• Provide counseling and support regarding end-of-life care.
• Collaborate with interdisciplinary teams to ensure holistic patient care.

Requirements

• Minimum of a Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong knowledge of social work theory and practice.
