SOCIAL WORKER PRN

Job Description

We are currently looking for a dedicated Social Worker PRN to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.

Responsibilities

• Provide clinical social work services to patients/residents and families.
• Assess common problems and provide counseling related to various issues.
• Assist in discharge planning and end-of-life counseling.

Requirements

• Basic professional knowledge with a Bachelor's degree.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
