Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health. As a Social Worker, you will provide crucial clinical social work services and psychosocial assessments to patients, residents, and their families. Your responsibilities will include addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling on end-of-life issues.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address common problems related to aging, family dynamics, and healthcare transitions.
• Offer counseling and support for end-of-life planning and coping.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
