SOCIAL WORKER PRN

Job Description

We are currently looking for a passionate PRN Social Worker to join our dedicated team at LifeBridge Health. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families. Your role will involve addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments and provide counseling services.
• Help patients and families cope with medical diagnoses and procedures.
• Assist with discharge planning and address end-of-life issues.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
