SOCIAL WORKER PRN

Job Description

We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various common challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and intervene with patients and families.
• Provide counseling and support related to medical conditions and end-of-life issues.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Participate in discharge planning and facilitate referrals to community resources.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience as a licensed social worker (LBSW/LMSW/LGPC).
• Strong knowledge of social work theory and practice.
