Job Description

We are looking for a passionate and dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing a variety of common issues. Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address common problems such as adjustment to institutional living, financial concerns, family relationships, and powerlessness.
• Offer counseling related to various aspects such as medical conditions, aging, discharge planning, and end-of-life care.

Requirements

• Bachelor's degree and 1-3 years of experience in social work.
• Licensed as LBSW, LMSW, or LGPC.
• Strong knowledge of social work theory and practices.
• Ability to work with diverse populations and demonstrate empathy and compassion.
