Job Description

We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a PRN Social Worker at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges they may face. Your role will involve supporting individuals with adjustment to institutional living, financial concerns, family relationships, medical procedures, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Provide clinical social work services to patients and families.
• Conduct psychosocial assessments and address common psychosocial challenges.
• Support individuals in coping with medical diagnoses and end-of-life issues.
• Collaborate with healthcare teams to develop comprehensive care plans.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience as an LBSW/LMSW/LGPC.
• Strong understanding of social work theory and practice.
• Ability to work with individuals and families facing various social and emotional challenges.
