SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our compassionate team at LifeBridge Health. In this role, you will provide essential clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to aging, death, and dying.

Responsibilities

• Deliver clinical social work services to patients/residents and families.
• Conduct psychosocial assessments to address various issues.
• Provide counseling on adjustment, family relationships, and end-of-life care.
• Collaborate with multidisciplinary teams for comprehensive patient care.

Requirements

• Bachelor's degree with relevant professional knowledge.
• 1-3 years of experience in the field.
• LBSW/LMSW/LGPC certification.
• Strong communication and interpersonal skills.
