SOCIAL WORKER PRN

Job Description

We are currently seeking a passionate and dedicated Social Worker (PRN) to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing various challenges and needs.

Responsibilities

• Offer clinical social work services to assist patients/residents and families in coping with a range of issues.
• Conduct psychosocial assessments and develop appropriate care plans.
• Address topics such as adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling on end-of-life matters.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
