Job Description

We are looking for a passionate and dedicated Social Worker PRN to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients/residents and families.
• Address issues related to adjustment to institutional living, financial concerns, and family relationships.
• Offer counseling related to death and dying.
• Participate in discharge planning.

Requirements

• Basic professional knowledge with a Bachelor's degree.
• 1-3 years of experience in social work.
• LBSW/LMSW/LGPC certification.
