Job Description

We are looking for a dedicated PRN Social Worker to join the LifeBridge Health team at Levindale Hospital in Baltimore, MD. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, and more.

Responsibilities

• Provide psychosocial assessments to patients and families.
• Address concerns related to medical diagnoses, aging, powerlessness, and end-of-life planning.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
• Participate in discharge planning and counseling services.

Requirements

• Bachelor's degree and LBSW/LMSW/LGPC certification.
• 1-3 years of relevant experience in social work.
