JOB SUMMARY: Provides clinical social work services or psychosocial assessments to patients/residents and families, regarding common problems:
- Adjustment to institutional living
- Financial concerns
- Family relationships
- Difficulty understanding/accepting medical/diagnoses/procedures/treatments
- Changes relating to aging
- Powerlessness
- Discharge planning and counseling related to death and dying
REQUIREMENTS:
- Basic professional knowledge; equivalent to a Bachelor's degree; working knowledge of theory and practice within a specialized field
- 1-3 years of experience
- LBSW/LMSW/LGPC
