Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide essential clinical services and psychosocial assessments to patients and their families, addressing a range of common challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments to identify patient needs.
- Provide counseling and support to patients and families facing difficult circumstances.
- Assist with discharge planning and end-of-life care discussions.
- Collaborate with a multidisciplinary team to ensure comprehensive patient care.
Requirements
- Bachelor's degree and 1-3 years of relevant experience.
- Licensed as LBSW, LMSW, or LGPC.
- Strong interpersonal and communication skills.
- Ability to work effectively in a healthcare setting.
