Job Description

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address issues related to adjustment to institutional living, financial concerns, and family relationships.
• Offer counseling on acceptance of medical procedures, changes related to aging, and end-of-life planning.
• Collaborate with healthcare teams to support patient well-being and discharge planning.

Requirements

• Bachelor's degree or equivalent with 1-3 years of experience.
• Licensed as LBSW/LMSW/LGPC.
• Strong knowledge of clinical social work theory and practices.
• Compassionate and empathetic approach to patient care.
