SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker on a PRN basis to join our team at Levindale Hospital in Baltimore, MD. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing various challenges related to institutional living, financial concerns, family dynamics, medical procedures, aging, and end-of-life care.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and their families.
• Assist with discharge planning and support patients in adjusting to institutional living.
• Work with interdisciplinary teams to ensure comprehensive care for patients.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience as a Social Worker.
• Licensed as LBSW, LMSW, or LGPC.
• Ability to demonstrate empathy, communication skills, and ethical conduct.
