SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges they may face. Your responsibilities will include assisting with adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling related to death and dying.

Requirements

• Bachelor's degree or equivalent professional knowledge.
• 1-3 years of experience.
• LBSW/LMSW/LGPC certification.
