Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families. This includes addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to medical treatments and end-of-life care.
Responsibilities
- Conduct psychosocial assessments and provide counseling to address patients' and families' needs.
- Assist with discharge planning and support patients in adjusting to their new living situations.
- Collaborate with healthcare professionals to ensure comprehensive care for patients.
- Advocate for patients' rights and assist in navigating the healthcare system.
Requirements
- Bachelor's degree in social work or related field.
- 1-3 years of social work experience.
- Licensed as LBSW, LMSW, or LGPC.
- Strong communication and interpersonal skills.
