Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will be responsible for providing clinical social work services and psychosocial assessments to patients, assisting them with various challenges and adjustments they face during their care.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and their families.
- Assist patients with financial concerns, discharge planning, and coping with medical issues.
- Collaborate with healthcare teams to support patient needs.
Requirements
- Bachelor's degree with 1-3 years of relevant experience.
- LBSW, LMSW, or LGPC certification.
- Strong knowledge of social work theory and practice.
