Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, their families, addressing a range of common problems. This position requires a compassionate individual who can assist with adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.
Responsibilities
- Conduct psychosocial assessments of patients and families.
- Assist individuals with adjustment to institutional living and aging-related changes.
- Provide counseling and support regarding medical procedures, discharge planning, and end-of-life care.
Requirements
- Bachelor's degree with 1-3 years of experience.
- LBSW/LMSW/LGPC certification.
- Familiarity with clinical social work theory and practice.
