SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker (PRN) to join our team at LifeBridge Health in Baltimore, MD. This role involves providing clinical social work services and psychosocial assessments to patients/residents and families, addressing a range of common challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address various challenges faced by patients/residents and families.
• Work on adjustment to institutional living, financial concerns, family relationships, and more.
• Collaborate with interdisciplinary teams for comprehensive care.
• Participate in discharge planning and support families in difficult times.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW/LMSW/LGPC.
• Strong communication and interpersonal skills.
• Ability to work with diverse patient populations.
