SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker PRN, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing a variety of common challenges such as adjustment to institutional living, financial concerns, and discharge planning.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients/residents and families.
• Address issues such as adjustment to institutional living, financial concerns, and family relationships.
• Assist with understanding and acceptance of medical diagnoses, procedures, and treatments.
• Support individuals dealing with changes related to aging, powerlessness, and end-of-life counseling.
• Contribute to discharge planning processes.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong knowledge of social work theory and practice.
