SOCIAL WORKER PRN

Job Description

We have an exciting opportunity for a PRN Social Worker at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a variety of challenges related to institutional living, financial concerns, family dynamics, medical procedures, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Offer clinical social work services to patients and families.
• Assess and address common psychosocial challenges.
• Provide counseling and support related to various life transitions and medical conditions.
• Collaborate with healthcare teams to ensure comprehensive care for patients.

Requirements

• Bachelor's degree with 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Demonstrated knowledge of clinical social work practices.
• Strong communication and interpersonal skills.
