SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and skilled Social Worker to join our team at LifeBridge Health. This position is PRN (as needed) based in Baltimore, MD, at Levindale Hospital. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to medical procedures and end-of-life care.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Assist patients/residents and families with various issues including adjustment, financial concerns, and discharge planning.
• Offer counseling and support related to aging, end-of-life care, and other emotional challenges.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
