SOCIAL WORKER PRN

Job Description

We are seeking a compassionate Social Worker to join our team on a PRN basis at LifeBridge Health in Baltimore, MD. As a member of our hospital's social work department, you will provide clinical services and psychosocial assessments to patients and families, addressing a range of common challenges related to medical procedures, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services to patients and families.
• Address issues such as adjustment to institutional living, financial concerns, and family relationships.
• Offer counseling and support related to medical diagnoses, aging, and end-of-life care.
• Participate in discharge planning and assist with navigating complex healthcare systems.

Requirements

• Minimum Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong interpersonal and communication skills.
