SOCIAL WORKER PRN

Job Description

We are currently looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients and their families, addressing various challenges they might face during their stay in the hospital.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and families.
• Address issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life care.
• Provide counseling and support related to medical treatments, aging, discharge planning, and bereavement.

Requirements

• Bachelor's degree in Social Work or related field
• 1-3 years of relevant experience
• LBSW, LMSW, or LGPC certification
• Strong understanding of social work theory and practice
