SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker to join our team at LifeBridge Health. As a Social Worker PRN, you will provide clinical social work services and psychosocial assessments to patients, addressing a variety of common issues related to institutional living, family relationships, medical procedures, aging, discharge planning, and end-of-life care. This role offers the opportunity to make a meaningful impact on the lives of our patients and their families.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients/residents and their families.
• Assist individuals in coping with emotional, financial, and social challenges.
• Collaborate with healthcare teams to develop effective treatment plans and discharge arrangements.

Requirements

• Bachelor's degree and LBSW/LMSW/LGPC certification.
• 1-3 years of relevant experience in clinical social work.
• Strong communication and interpersonal skills.
• Ability to work effectively in a fast-paced healthcare environment.
