Job Description

We are looking for a passionate PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, focusing on various aspects including adjustment to institutional living, family relationships, and discharge planning.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and their families.
• Address common problems related to medical procedures, aging, and end-of-life care.
• Develop discharge plans and coordinate care with other healthcare professionals.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of experience as a Social Worker.
• LBSW, LMSW, or LGPC certification.
