SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join the LifeBridge Health team at Levindale Hospital in Baltimore, MD. In this role, you will provide clinical social work services and perform psychosocial assessments to patients, assisting them with various challenges they may face during their healthcare journey.

Responsibilities

• Provide counseling and support to patients/residents and their families dealing with a range of psychosocial issues.
• Assist individuals in coping with adjustment to institutional living, financial concerns, family relationships, and end-of-life decisions.
• Conduct discharge planning and offer bereavement counseling.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience as a licensed social worker (LBSW/LMSW/LGPC).
• Strong working knowledge of social work theory and practice.
