Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, assisting with various challenges and adjustment needs.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Address common issues such as adjustment to institutional living, financial concerns, and family relationships.
• Assist in discharge planning and provide support related to end-of-life care.
• Collaborate with healthcare professionals to ensure the well-being of patients.

Requirements

• Bachelor's degree and LBSW/LMSW/LGPC certification.
• 1-3 years of relevant experience.
• Strong knowledge of social work theory and practice.
• Excellent communication and interpersonal skills.
