Job Description

We are looking for a skilled PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and more. This role plays a critical part in supporting individuals through difficult times and ensuring their well-being.

Responsibilities

• Conduct psychosocial assessments for patients and families.
• Offer counseling and support for issues related to medical diagnoses, aging, and end-of-life care.
• Assist in discharge planning and provide guidance on community resources.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Compassionate and empathetic approach to patient care.
