Job Description
We are looking for a motivated and compassionate Social Worker PRN to join our dedicated team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges and supporting their well-being.
Responsibilities
- Offer clinical social work services and psychosocial assessments.
- Support patients/residents and families with adjustment to institutional living, financial concerns, family relationships, and other common issues.
- Address challenges related to understanding medical procedures, aging, discharge planning, and counseling on death and dying.
Requirements
- Bachelor's degree and 1-3 years of experience.
- LBSW/LMSW/LGPC certification.
- Strong knowledge of social work theory and practice.
