SOCIAL WORKER - CARE MANAGEMENT - CARROLL HOSPITAL

Job Description

We are looking for a dedicated Social Worker to join our Care Management team at Carroll Hospital, part-time with a weekend commitment. In this role, you will collaborate with the healthcare team to ensure efficient care delivery for inpatients and engage with community agencies addressing various social issues. If you have a Master's degree in social work, 2-4 years of experience, and a passion for making a difference in people's lives, we want to hear from you.

Responsibilities

• Partner with healthcare team members to coordinate patient care effectively.
• Address social issues such as abuse, neglect, substance abuse, and mental health.
• Link patients to necessary community resources and financial assistance.

Requirements

• Master's degree in social work.
• 2-4 years of social work experience, preferably in a medical setting.
• LCSW-C license preferred; LMSW licensed candidates will also be considered.
