SOCIAL WORKER- BALTIMORE COUNTY/CITY

Job Description

We are looking for a dedicated Social Worker to join our team at LifeBridge Health in the Baltimore County/City area. As a Social Worker, you will play a crucial role in providing professional and compassionate services to individuals in their preferred residence. You will assess psychosocial, environmental, and financial needs, collaborate with patients, families, and interdisciplinary teams to develop care plans, and act as an educator and consultant. Your expertise will contribute to evidence-based practices and ensure optimal patient care.

Responsibilities

• Assess patients' and families' psychosocial, environmental, and financial needs.
• Develop, implement, and evaluate care plans in collaboration with patients, families, and interdisciplinary teams.
• Act as an expert practitioner, educator, and consultant.
• Ensure adherence to Fleet Safety Requirements.

Requirements

• Master's Degree in Social Work from an accredited institution.
• LCSW certification required.
• Strong interpersonal communication skills.
• Ability to maintain accurate documentation records.
