Position: CSC Case Manager

Job Summary: The CSC Case Manager at PermiaCare will be responsible for providing psychoeducation, psychosocial rehabilitation, and case management services to individuals enrolled in the Coordinated Specialty Care-First Episode Psychosis (CSC-FEP) program. This role involves working closely with a multidisciplinary team to support individuals between 15 to 30 years old with their mental health, employment, and educational needs.
Key Responsibilities:
- Provide direct services and support to individuals as per recovery plans and assessments.
- Collaborate with individuals and families to achieve recovery goals.
- Ensure required service delivery and documentation meet program standards.
- Offer education and outreach on First Episode Psychosis to various stakeholders.
- Coordinate integrated services and transportation for individuals.
Qualifications:
- Bachelor's degree in psychology, social work, or related field.
- Minimum of 1 year experience in social services preferred.
- Valid Texas Driver's license and QMHP certification within 6 months.
