ABOUT APM
Asociación Puertorriqueños en Marcha (APM) is a nonprofit organization in Philadelphia, PA providing human services to the community since 1970. We are currently looking for a Community Connector to perform community outreach focusing on victims of non-fatal shootings and individuals at risk of perpetuating violence in North Philadelphia.

General Summary:

The Community Connector will collaborate with APM's Violence Intervention team to engage with targeted individuals and provide essential support services as part of our violence intervention program.

Responsibilities:

  • Engage with non-fatal shooting victims and at-risk individuals identified through various channels.
  • Consistently meet service criteria for eligible individuals within defined timelines.
  • Represent APM at community events and engage with key partners to enhance program effectiveness.
  • Maintain accurate outreach lists and conduct regular follow-ups with program participants.
  • Conduct eligibility assessments and work closely with supervisors to coordinate program activities.

Requirements:

  • BA preferred or equivalent experience with a minimum of three years working with at-risk youth.
  • Experience with street-involved high-risk youth.
  • Valid Driver's License or access to public transportation.
