Job Description
We are looking for compassionate and dedicated individuals to join our dynamic Case Management team at Pinnacle Services in Hennepin County. As a Case Manager, you will have the opportunity to work with individuals on waivers for developmental disabilities and other conditions, ensuring they receive the support they need to thrive.
Responsibilities
- Assess the needs of persons served and coordinate appropriate resources.
- Monitor service delivery and collaborate with providers to meet client needs.
- Develop and implement service plans in conjunction with interdisciplinary teams.
- Document client-related work for billing purposes.
Requirements
- Bachelor's degree in social work or related field.
- Experience in social work, case management, or care coordination.
- Valid driver's license, vehicle, and insurance.
- Ability to pass background checks.
