Job Description
Great Falls Clinic is seeking a dedicated Admission/Discharge Nurse to provide exceptional care to our patients. As a key member of our nursing team, you will work under the direction of healthcare providers and nursing leadership to deliver comprehensive patient care. This role involves assessing patient needs, planning interventions, implementing care plans, and evaluating outcomes to ensure the highest standards of nursing practice. The Admission/Discharge Nurse will collaborate with interdisciplinary teams to optimize patient care delivery and support patients and their families throughout their healthcare journey.
Responsibilities
- Complete medication reconciliation, admission health history, and medication education for new admissions and discharges.
- Review and ensure completion and continuity of admissions/discharges.
- Provide support to the Med/Surg floor based on census needs.
- Direct and coordinate activities of nursing and allied services staff.
Requirements
- Graduate of a Nursing Program with MT RN licensure.
- Certifications: ACLS, PALS, BLS (to be completed within 6 months of hire).
- Medical/Surgical nursing experience and IV skills are essential.
- Proficient in basic office skills.
