Schedule: 4/10-hour shifts weekly, 8:30am-7:00pm
Summary: The Inpatient Care Manager works collaboratively with the clinical team and medical provider to coordinate discharge planning and interventions for patients. The role focuses on promoting patient wellness, improving care outcomes, optimizing health service utilization, and reducing payment denials for patients with complex health needs.
Requirements: Basic professional knowledge; Bachelor's degree or equivalent; BSN required from CCNE accredited schools (exception for internal candidates with specific roles).
