In this role, you will play a vital part in the Resident Assessment Process by completing key items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Position Highlights:

• Direct and oversee the RAI process in the nursing center.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Train team members on clinical content and electronic completion of MDS items.

Requirements:

• LPN in good standing with current state licensure.
• Prior experience in hospital, long-term care, or healthcare facility.
• Proficiency in general nursing practices and MDS completion.
• Computer skills, AccuMed experience preferred.
• Previous MDS Coordinator experience desirable.
