INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As an Inpatient Care Manager, you will collaborate with the clinical team and medical providers to coordinate discharge planning and interventions for patients with complex health needs. Your role will focus on promoting patient wellness, improving care outcomes, ensuring efficient health services utilization, and minimizing payment denials.

Requirements
