Job Description
Join Heartland Home Health as a RN Case Manager and be part of a leading hospice, palliative, home health, and personal home care provider within the Gentiva family. As a Home Health RN Case Manager, you will deliver skilled nursing care to clients in their homes, offering vital support and guidance to both patients and their families. Working under the direction of the Clinical Manager, you will ensure that all services provided align with physicians' orders and meet the highest standards of care.
About You
- Hold a current RN license in good standing in the applicable state.
- Possess a minimum of 6 months of recent institutional nursing experience, unless waived after a personal interview.
- Demonstrate extensive experience for RNs providing skilled intervention for PASSPORT patients.
- Capable of daily travel up to 100% as part of the role.
