Overview:

• Directly report to the Administrator, Executive Director, or Patient Care Manager Senior.
• Provide direct patient care services and supportive care to patients and their families.
• Manage an interdisciplinary group to enhance the quality of hospice care.
• Evaluate patient/family needs and assist in the execution of care plans.
• Maintain patient clinical records and participate in quality assessment programs.

About You:

• Compassionate, empathetic, and kind towards patients and families.
• Hold a current RN license or have the ability to obtain one.
• 1+ years of nursing experience in hospice, home health, or oncology preferred.
