Responsibilities
- Conduct initial and ongoing assessments of patients' physical and psychosocial status.
- Work with interdisciplinary teams to develop and implement personalized care plans.
Requirements
- Graduate of an accredited Registered Nurse (RN) program.
- Active, unrestricted Registered Nurse (RN) license in the State of South Carolina or compact multi-state license.
- Minimum of twelve months of recent nursing experience.
- One year of hospice care experience preferred.
