Job Description

Cassia, a faith-based, nonprofit organization with a mission to foster fullness of life for older adults in the spirit of Christ's love, is seeking a Full-Time Licensed Social Worker (LSW) to join Redeemer Health Care Center. In addition to a competitive salary, we are offering a $5,000 Bonus. This role involves advocating for residents in long-term care and their family members to ensure they receive high-quality care and services. Responsibilities

• Provide valuable social services to residents and their families in medical, emotional, physical, or psychological concerns.
• Coordinate social services for the facility, individualized plans for the residents, medical equipment, counseling, or other follow-up treatment plans.
• Act as an advocate ensuring basic human and resident rights.
• Conduct and coordinate admissions, provide appropriate referrals and discharge planning for residents.
• Provide resources and referrals to staff, residents, and family members.
• Additional tasks as required.

Requirements

• Must be a licensed Social Worker (LSW).
• Transitional or long-term care experience preferred with an opportunity for advancement.
• Knowledge of state and federal guidelines related to long-term care and resident rights.
• Mission-minded with excellent communication skills.
