Job Description
We are looking for a dedicated and compassionate Inpatient Case Manager (RN) to join our team at LifeBridge Health. As an Inpatient Care Manager, you will work closely with the clinical team and medical providers to coordinate discharge planning and interventions for patients with complex health needs. Your role will focus on promoting patient wellness, improving care outcomes, and ensuring efficient health service utilization.
Responsibilities
- Collaborate with clinical team and medical providers to facilitate discharge planning.
- Promote patient wellness and improved care outcomes.
- Ensure efficient utilization of health services.
- Minimize denials of payment among patients.
Requirements
- Bachelor's degree or equivalent professional knowledge.
- BSN required from CCNE accredited schools.
- Experience as a Utilization Review Nurse, Clinical Care Coordinator, Patient Care Integrator, or RN Case Manager is a plus.
